Working Without Working Visa

Hi everyone, I am a registered Psychologist from Australia and want to come and study Italian in Perugia for 1 year. I am too old to get any kind of working visa based on age. Would it be possible for me to set up a small private practice seeing expat clients without a working visa? I specialise in Anxiety, Depression and Adjustment issues so know I could add value. Do you know of other Psychologisrs or similar who may be doing something like this without the proper visa. Thank you.
