I thought I'd start a thread, hopefully amusing, about people making serious errors.
I'm not talking about dropping your mobile phone down the toilet; more throwing away millions because you don't know how to use a tape measure.

Two situations in Denver.
1.  They built a beautiful long (20+block or so) pedestrian zone and only transport was a shuttle buss system that ran up and down the  zone.  Great idea, but they forgot to measure the height of the train and the level of the old style street lamps along the route....just 6 inches off and on the first run the broke all the street lamps  and had to add height to all of them.

2.  At the new beautiful international airport for the cost of millions they built an automated baggage system from a German company.  Purpose was to take you bag from check in to the plane and then from incoming flight to the terminal.   Get rid of the old tug (pull vehicles) and luggage cart system.... Well a few million down the drain.  The system was called  BAE and renamed Bags Are Everywhere system to the locals. Needless to say after many attempts  ($$$$) to fix the system they are back to tugs and baggage carts like almost all airports in the world.

In Belgian papers call your example "the French Fyra". Read "End of service" on http://en.wikipedia.org/wiki/Fyra for more details.

The same situation in the Netherlands: the National Train Company bought the Italian Frya for millions and they  caused a lot of problems.
In this case the Belgians are more clever then the Dutch: they cancel the purchases.
