If spouse:
(a) Marriage Certificate
(b) Birth certificate
(c) Passport with a tourist or business visa
(d) Medical exams
(e) Fee of Rs5,000
(f) 4 passport photos
Bring all that to the BOI / PIO (4th floor, Sterling House, Port Louis). Fill the form and submit the application. The main applicant should already have his permit in hand prior to making dependent application.
In about 10 days, the dependent permit is issued.
Same for children, but without marriage certificate (of course)
