Ishaar registration form

Hey guys, i hope you all are doing great.
here, i need a help,
can anyone provide me information regarding Dependents Registration Form for Ishaar, so every time i dont need to visit the passport office and can print the Exit Re Entry form from home or office,
I tried a lot but Unfortunatly can not find any form.
Help is highly Appreciated.

I don't think anyone must have the form :) Nor they'll accept a scanned+printed form.

You'll have to visit one of following (once only):

Registration centers
====================
1 – Passports department Building -Riyadh - Makkah - Madinah - Jeddah - Dammam
2 - Passports Service centers  available in malls  in Riyadh (Sahara Mall - Flamengo Mall ) and in Jeddah (Tahlyah Mall).
2 - The e-Gate Office in the King Khaled International Airport - International Terminal

So is it like that we need to go and fill up the form there and not online??
