Usually if you meet the requirements for renewal, you can.  Go ask the police who issued it in the first place, they will give you an answer.

It will depend on how long your current permesso has been expired.  As the previous poster said, you can ask the Questura, but if your permesso has been expired for too long, they can deny it's renewal.

You should renew your Permesso before it has expired, but you may have a period of grace. Go to your nearest Questura and ask them to help you.
