I had Cigna through my job in the states. I had a great experience with it. I am not sure if it varies depending on location.
Triple S is quite popular here.
Tripple S is part of Blue Cross and Blue Shield.
MCS is also popular and Humana but triple S is the most popular so far.

If you google Puerto Rico Health Insurance Conference 2017 it will lead you to the announcement of the conference that will be on March 9. The poster lists health care companies.  I have heard good things about MMM (Medicare y Mucho Mas) but do not have first-hand experience.  Last time I checked their plans started at $30 a month.
