If you mean a Non-Immigrant O extension based on marriage to a Thai spouse, I wouldn’t assume that health insurance is now required just because some retirement visa routes have an insurance requirement.

The requirements can differ depending on the visa/extension route, so it’s worth checking the current requirements with the Immigration office handling your extension before buying a policy just for visa purposes.

Health insurance can still be worth considering for your own medical cover, of course, but that’s a separate question from whether your particular extension requires it.
