Hi, I live in Bali, what I can say Is that foreigner are generally allowed to get the vaccine showing Kitas or Kitap with SKTT or KTP with a NICK.
To the link below u can find a list of places where Is available the vaccine in Jakarta, Google them one by one to get the phone number and ask them.
Good luck and keep us updated.

No, mate. At the moment, only certain foreigners could get access to it. In Jakarta, without NIK, you'll get rejected everywhere.
What about you in Bali? Have you got one?

Should be the same everywhere, all foreigner with Kitas or Kitap with a NICK can be vaccinated, but seems its not.
In Bali generally need  a NIK, but some faciliities dont ask fot that, Kitas or Kitap Is enough, they can register u manually.
Im on list  for Astrazeneca with an hospital, they ask for a NIK.
