Applying for residency from California, U.S.A.

Has anyone recently obtained residency who lived in California (a US citizen)?  I'm confused by the instructions from the Consulate, specifically about proof of health insurance and a lease on a rental or house purchase which accompanies the application.  When did you arrange insurance and a residence, since the application is made in the US?  After submitting your application, did you wait in California or go straight to Portugal?  I'm applying for a visa as a retired person.
