This site compares the three most popular plans in Fortaleza, and can give you a quote for any or all of them.  Note that it's an agent site; if a plan interests you,  you may be able to get a slightly better quote by going directly to the plan's sales office:

When I was looking for a plan, Amil was not accepting any new clients older than 62; that may be different now.   Plan reputations vary greatly by locality, so it's worth checking with local friends and acquaintances on how a plan is viewed locally.

If you don't currently have a private health plan, you can expect to go through a one year "carência" or probationary period, during which more and more services will gradually be made available to you.  If you already have a plan and have completed a "carência" there, the requirement should be waived by the new carrier.

At 75, my guess is that your monthly payment would be in the R$1000 - R$1500 range, depending on coverage.  Since you're over 60, going forward you should only have the annual increase that ANS, the federal agency that regulates private health plans, allows to all plans based on their actual cost experience, without the additional age-based increase that clients under 60 also have.
