If you are selling your vehicle to a private party does the Bahrain Traffic department issue a document to the seller that the car has been transferred from the seller to the buyer?

Don't think so.  At least I didn't get anything when I transferred my old car.   All that happened was that the car was no longer appearing under me in the online traffic portal on bahrain.bh.

If you transfer the insurance and the buyer backs out, do you just call the insurance co to revert it back?

That may work, if not they may ask you to visit a branch.
