You can start looking at "rede medica" by the private healthcare site like here, for advance care.

You will probably find similar address book at fidelidade

I would assume that the embassy of your passport country has a list of medical practitioners in Porto. Maybe consult this list first. Also, Private Hospitals usually show which languages the doctors working for them speak.
