Legally owning a used car in Sicily as a non-EU citizen

The very short answer is 'no' - you cannot do it legally.  But at this point you enter in to the italians relationship with the law, and whether its worth the risk. 

There is a company  TARGAITALIA that do it. They are on Instagram.

Looks like a scam to me - Give them 69USD and then they - with no address or way of tracing them - will do what exactly?   

@Modicasa That was not my experience when I registered a car and a trailer i brought from the UK. targa.italia is the profile on Insta. no fees.

Im glad it went well for you -  Presumably you imported your vehicle prior to December 2021? 

@Modicasa I imported the trailer and car in late '22 early '23 from the UK, import process was a overly complicated but I found a broker that smoothed it over.

I contacted the Targa Italia people after getting a recommendation from an exhibitor at a car show in Milan.

The trailer was registered within 1 month, and the car about 3-4 months if I recall. Fees, made sense.

Its funny you mention the 69E charge on their web site, I didn't pay anything but for the service, but I remember asking about that charge and the guy told me that it was because of covid. They were getting 100's and 100's of unproductive calls per week and that was to weed out the information seekers. Anyway, my experience was good, you may know differently.
