"Demande de passeport" after receiving citizenship

Hello all.

I was hoping that someone might know what is the correct information to fill in when applying for a French Passport. I am Australian by birth and have just received my French Nationality, I am currently living in Australia and applying for here also. I have applied and received my Act de Naissance from the French administration. However, it is unclear if I am write my Australian information as is seen on my French Act de naissance, or am I to write where and when my citizenship was granted?  I am not sure if this is clear.
