Health insurance to cover you in US is going to be costly no matter what. You can get an international plan, but again if you cover the US the premiums will be 40% more.

We have coverage here in DR with Humano ($52/month) since this is where we live. It doesn't make sense for us to carry coverage in the US since we aren't there. When we travel back to the US, we purchase a short-term policy from IM Global.

If you are a US citizen, the vast majority of travel insurance policies will not cover you when you are in the US. IM Global has policies specifically for US citizens who live abroad but travel back to the US for vacations, family visits, etc.
