United Doctors are great and fluent in English.

There are only two hospitals here in town - the International Clinic (aka Cuban Clinic because the doctors are all Cuban born and trained) which is private, and the newly opened Hospital Municipal Pablo Antonio Paulino, which is a government-run, public hospital. With insurance, the International Clinic is the way to go! We have a friend who was there for over a week this spring and he received excellent care!
