Lost Birth certificate from Riyadh

Hello..I was born in Jubail, Saudi Arabia in 1988 and I have lost my birth certificate. I dont have a copy of birth certificate or any other proof with place of birth. Unfortunately, the place of birth mentioned in my passport is also wrong. Now I am residing in India and i am i need of my origibal birth certificate to move to US. Can someone please help me with how i can apply for my lost original birth certificate again from jubail.

Do you know any way which we can check out to obtain the missing birth certificate from jubail. I do Not have any other document stating proof of birth And I am in need of my certificate to migrate And go abroad.

I got mindle from one agent. He took 5000sr from me and get me my certificate. I born in Saudia 1990 and lost it. I am currently in Amsterdam. I need that for application of Dutch citizenship. 10 years back when I was in KSA i tried personally but not able to get. Spoke to some people recently the people by themselves went 50 times but no succees.

My daughter was born in 2017 in Riyadh, when they were on permanent family visa.

Now again i need to bring them back in family visit visa, so for stamping they need original birth certificate, which i lost it.

Last year in March 2021 I got my birth certificate.It was in RIYADH.

It was a long process which took 2-3 months:

The procedure for birth certificate are as follows:

Stage 1: Obtaining Letter from General Directorate of Ministry of Health

-Documents Required:

a)Copy of Mother Passport at time of birth.

b)Copy of Father Passport at time of birth

c)Copy of Child first passport in which birth was entered.

d)Copy of Latest Iqama of Mother.

e)Copy of Latest Iqama of Father.

f)Copy of Latest Iqama of child.

g)Birth notification(Certificate from hospital)

h)Power of Attorney to MOH stamped by Saudi embassy in respective country+Ministry of Foreign Affairs (If person outside KSA)

Stage 2: Birth certificate from Civil affairs( Ahwal Madni )

-Documents required:

a)Copy of Mother Passport at time of birth.

b)Copy of Father Passport at time of birth

c)Copy of Child first passport in which birth was entered.

d)Copy of Latest Iqama of Mother.

e)Copy of Latest Iqama of Father.

f)Copy of Latest Iqama of child.

g)Birth notification(Certificate from hospital) OR Copy of Birth Certificate

h)Power of Attorney to Civil Affairs stamped by Saudi embassy in respective country+Ministry of Foreign Affairs (If person outside KSA)

One of the documents mentioned in the forum is copy of the birth certificate or birth notification form. So I would like to know if you had the original birth notification form from the hospital made when you were born or did the hospital make it again for you when you recently needed it? Thanks! To anyone who lost its birth certificate you can follow up at your country origin birth registrar agency for sure it will forwarded from country embassy based KSA after an issued a passport for the Child.unless if the case is undocumented child birth thats an another case....under KSA law within 15 days after the child birth , it should register at Al-Ahwal Al-Madaniyah then Ahwal Madaniyah gave a Temporary Birth Certificate in order the child to issue a process  passport but it need to translate into English first, after that the parent can process the passport use the temporary birth certificate translated English. After 1-2 months the child should issue an IQAMA , after Completed 1year with completed vaccines doses the original birth certificate will be release by Ahwal Madaniyah and to show proof to bring the vaccinated card .The Passport Agency (embassy) role is to forwarded those passport documents (including Temporary Birth Certificate English Translated) to the country origin for the future need in case.
