This one’s actually easiest of all to get.

You can find a sample declaration online for free on many sites.  Just modify it, print it out and sign it in front of a notary.  Once you do this, just have it apostilled.

Modify it in what way I’m in Colorado so how would I modify it so it will get approved?

Who is telling you that you need this?

This is the form that several Brazilian Consulates (this one is from the website of the Consulate General in Chicago) accept from the non-Brazilian spouse when they register a marriage with a Brazilian.  As HaDov says, it would need to be signed, notarized, and apostilled.  It would also require a Sworn Translation for use in the cartório:

For the purpose of registering my marriage in Brazil, I, __________________________________ (name), born on____/____/____, in__________________________ (city/state), hereby declare that I have never married or divorced a Brazilian national.

         Date:   __________/________/__________ Signature:  _________________________________________________

Notary Public:

Please stamp, sign,
and indicate your State
and Comm. Expiration

If those are their instructions, that's what you should follow.

It's a good idea for you and your girlfriend to be aware of this list, as well:

So all I do is print that form and I’ll be okay too marry? Do you have the pdf file for that form?

No, I don't.  It just looks like a Word document that somebody at the Consulate ginned up -- copy the text into a Word document of your own, and you have as good as they do.

As for being "ok to marry", I suppose so, if that's what the cartório is telling your girlfriend, as long as you have your old marriage certificate and your divorce decree as well.

So a printed version of this then take it too get notarized then apostille it will be accepted by the notary?
