I never applied at a consulate so I'm not totally sure about the health insurance requirement, but from what I understand, you may be able to get a monthly-renewing insurance policy and cancel/switch to a local provider after you arrive. This way you don't have to pay advance the whole year, but most policies will require a minimum term anyway.

On the consulate route, the resolution (Art. 3º, II) requires health insurance valid in Brazil for your whole intended stay — so the question isn't really '12 months vs 364 days' in the abstract, it's whether the policy document shows coverage for the entire period you're declaring. Some consulates are strict about the validity dates printed on the certificate, so a 364-day plan is exactly the right shape, but confirm with your specific consulate that they'll accept those dates before you buy. (For the in-country/MigranteWeb route, the dossier doesn't list insurance at all — it's a consulate-route requirement.)
