changing uk car to portuguese

Please can anyone help me, so i drove from scotland to portugal as me and my portuguese girlfriend decided to move to her home. So i filled in all the form took the car for a test. Then made an appointment with IMT sorted. Then i go to fill out the DAV tacx form and thats where it stips as section H asks when i arrived and what customs department i went to and what the number i have ??????? ok so i didnt see any information about this, i called the local customs office and the lady said i should contact a company who will help with the form, sorry im not paying 500 euros just for help with two lines on a form.

Please say someone out there has experince with filling in a DAV form ( vehicle customs decleration)

You can fill in the DAV form in person at the customs office or online at the Customs Portal here:

Pay attention to the deadlines for doing this.
