Some remarks:

The NHS is not totally free. Public health insurance cover all medical cases, but will there be money to pay for surgeries, doctor checkups, lab tests, exams of any kind and medication for everyone? ...Here or anywhere in the world ? ...

The NHS has worked much better than it does now. A significant % of local people use private health services. 

NHS and social security are different concepts. You are not entitled to the Portuguese social security system, but If you work in the country, you have the duty to contribute to it, as a worker.

I strongly suggest you to have a private health insurance. But you can always request an appointment at the local NHS health centres and go to hospital emergency units.

We are finding Private Health Insurance is limited to those under 75 yr old. What do the rest of us do?

Thank you for the helpful information as usual. So the travel insurance (inuding Healthcare coverage) that i submitted for my d7 visa application is still enough for my sef meeting? Or i should issue a private one before the appointment?

@JohnnyPT i am done with the d7 visa, i got it, but people are saying that sef asks about a private insurance, is it true? Or i can just show them my travel insurance that includes health coverage as well?  (it expires on the sef appointment day)

I don't understand the reference to the NHS here, as being registered with the NHS requires a residence permit. Perhaps this makes sense for visa renewals...
