There's a question in the ESTA application about whether one has stayed more than the allowed three months in the US. My answer is that I have. On my last trip I was in the US a few days exceeding the three months cos I was in a mental hospital. Is that grounds to deny entry to US the second time?

Also there's a question about US point of contact. I don't know anyone in the US. How do I answer that question?

You apply for ESTA and answer all questions truthfully. Then you apply for B1/B2 and bring documentation such as statements from physicians that medical treatment required your overstay plus the usual binding ties.
