Jobs are scarce in Ecuador. Also, the health sector is not highly paid, assuming a foreigner could even get a job in Ecuador in that field (it's not a given since priority for jobs is given to Ecuadorian nationals).

The government of Ecuador sets salaries in most areas of work. This article talks about world-wide salaries for doctors. For Ecuador the range is about 10K to 26K dollars per year:

Of course if you're not a doctor you would earn even less.

This article from 2017 notes that the salaries of doctors have gone up from that high of about $26K to as much as $4K/month, or $48K per year, depending on the specialty:
