If long-term care insurance figures in your planning, be sure to confirm with your carrier if you haven't already that your coverage can be applied outside the United States.  I found when I was planning my move that mine would not. 

With respect to accessing your retirement funds if you become incapacitated, that depends on the arrangements that you have now and the nature of the impairment.  If, like some expats, you're having your entire retirement income deposited in your Brazilian bank account, there's no reason that you can't keep accessing it through your banking app as you do now.  If, like some others, you collect your income in a US bank account and have regular transfers made to your Brazilian bank account then again, you should be able to continue that with only occasional hiccups.  The key is to maximize what can be done electronically, and minimize the need for physical activity and intervention.  The above assumes that any incapacity is physical; an expat who begins suffering cognitive impairment will need to take additional measures and precautions.
