Lost Driver's License

I've lost my PR driver's license and want to replace it. I vaguely remember the procedure to get the original. Can anyone walk me through the process and the documents needed. Also, can I go back to the office in Aguadilla to get or is there some other place I must go? Thanks in advance for your help.
