E104 form

Hi everyone.

I am leaving Malta soon and need to provide evidence in Germany that I was state-insured for the last 12 months in order to get a state insurance in Germany.

I've been told that I would need a E104 or S041 form from Malta.

Does anyone know what it is and where I can request it?
