Normally commune (cityhall) needs:
For the main applicant (WP holder):
- Work permit (optionally employment contract too)
- Copy of passport, visa (D)
- Registered rental contract of your accommodation in Antwerp
- Birth certificate (sometimes asked) apostilled
- Medical Insurance (sometimes asked)
For dependents
- Copy of passport, visa (D)
- Certificate of proof of relationship (marriage certificate) - apostilled
- Birth certificate - aspostilled
- Medical Insurance (sometimes asked)
Plus photographs, application form to fill, etc. -- Cityhall will tell you.
