It's a few steps from and on the same side with Petrolimex (which in turn is located at the corner of Nguyễn Văn Trỗi + Huỳnh Văn Bánh). If you're coming from Q1 or Q3, move over to the right hand side after you go over Công Lý Bridge.
You can see its sign at the top of the building from half a block away.
Yellow fever shot needs to be done at least 10 days before the departure date. I would visit the center ASAP just in case the vaccine is not available when you come in (it has happened before).
