Citizenship by Registration / Affidavit

Hello Malta Forum people.  I'm Kevin, I was lucky enough to be born in Malta and have citizenship / passport.  My wife will apply for citizenship by registration at the Citizenship Unit in Valletta.  My question is about the Affidavit - does this need to be prepared beforehand with a lawyer, or can we walk in with the blank form?  Anyone know what wording should be on the form?

@kdpritchard An affidavit is a sworn statement put in writing. You must do it in front of witnesses while also having it notarized at the same time.

What I failed to see was the Affidavit in among Form B :-)

We went to the office on Old Hospital Street this morning at about 0715. We filled in and signed  the form in the presence of the desk officer, €10 each.  We were out of there before 0815.

I believe the office is open 0730 to 1130 Monday, Tuesday, Thursday and Friday.  They were most helpful in person and by email.
