First off, the tortured English in the above sentence gives us no indication you would have the language skills necessary to deal with espanol and its subset, Spanish medical terminology.

A foreign national getting a salaried job in Ecuador from a local employer has historically been a tough proposition.

Especially so in this case since you would need to apply for a visa permitting you to accept a specific position (apparently not yet offered), a dubious proposition if you're 11,000 miles away from a potential sponsoring hospital or other employer.
