Hi foreigners can't just set up as doctors here. Those that I have net here work in teaching hospitals and are highly experienced.

There are always vacancies around the country for medical specialists at private hospitals. You could check out the English language newspapers: Staronline and New Straits Times. However, the doctors are self employed, although they operate through a hospital infrastructure.
Government Hospitals only employ Malaysian doctors. You would need to get a licence to practice from: http://mmc.gov.my/v1/index.php?option=c … &Itemid=50 Info available at this site.
There is a returning professionals programme, so they take precedence in any posts. There are quite a lot of locums required - just google and find some sites.
There are some jobs at Universities as lecturers.
