Public Health Permit for Customs Importation

Anyone know where I can get a Public Health Permit for customs importation ?

They are currently holding my parcel in Santo Domingo and I need to get this paperwork figured out for UPS.

She is a Customs broker who works in SD and can give you info.

Thanks for following up.

If it doesn't get cleared tomorrow then I will give her a call to figure out what's going on. I followed up with UPS customer service three times until I got a rep who helped me to get that info. Sounds like the person receiving the parcel needed to call the local UPS in Santo Domingo.

The status has now changed since I Whatsapped with him this morning so we will see. Would be a shame for them to not get their parcel. Especially since they just had a baby and we were able to include lots of items that will definately help them and baby out !!
