Indonesian Immigration Law UU6-2011
Article 8 states that:
(1) Every people who enter or exit the Indonesian
Territory shall be obliged to have a legal Travel
Document and still valid.
(2) Every Foreigner who enters the Indonesian Territory
shall be obliged to have a legal Visa and still
valid, unless determined otherwise under this Law
and under international treaty.

At least 2 blank pages lukereg plus one blank page if you are processing stamp, itas online at immigration,converting visa stamp outside indonesia to itas like visa 312,317,319,etc.
