You need to find a doctor who will support an application for a practice license and be a supervisor for a period of time.

Private hospitals here work on the basis of doctors running their own clinics on hospital premises and paying the hospital for central and shared services.
You would need to set up a business in Malaysia to do the above with various capital requirements plus your own insurance coverage
