We are bringing our small dog to the DR from the US. I read that he needs a "pet passport". Is that just the health certificate and vaccination information or is there something more to that?

Nope he just needs the international health certificate and proof of recent rabies vaccination. We fly our dog back and forth regularly .

What is the difference between an international health certificate and a regular one? Can any vet provide it or do we need a special form? We get a health certificate for him when flying within the state's. Is it different?

Yes the international certificate is different. Most vets will have this. The one for the US travel will not work for travel to the DR
