My name is Anne. I'm Thai and I work in a Consultant Company. Now I'm surveying about using services in the hospital in Bangkok of EXPAT. Therefore I would like you to help me to fill in the questionnaire but the questionnaire must be filled in in hard copy.

Anyway, I have to ask you some questiones.

Do you stay in Bangkok more than 6 months?
Do you have a job in Bangkok either part time or full time?
Within past 1 year, have you yourself used hospital service?
If yes, the hospital had you used must be one of these following hospitals: Bamrungrad, Samitivej, BNH, Bangkok
