What are the formalities to obtain a birth certificate?

What is the procedure to follow if both parents are of the same nationality or for a mixed couple?

A baby born in the United States and its territories (sovereign soil to include embassies and military bases) are considered immediately United States citizens.

United States hospitals will do the paperwork in connection with the birth certificate and social security number for the baby. Therefor no action is required at birth other than the parents proving their identity, usually proved health insurance but a parents birth certificate may be required.

As well as registering the birth in the USA, it is the parents' responsibility to register the birth at their home country embassy. The baby could essentially be a citizen of both the United States and another country depending on the parents home countries recognition of dual citizenship.
