Documents required for dependant Visa (spouse) for Belgium

Hello

My husband's work permit for Belgium is under process and he has initiated dependant Visa for me and our 8 months old as well . I am working here so most probably will be going after taking leaves from my current employer .

Request you to suggest the documents required from me and my daughter for dependant Visa of Belgium .I heard that birth certificate is also a mandatory document.  Please throw some light on that as well .can 10th class certificate work as a date of birth document? I am 1986 born

Also if I am not able to arrange for my birth certificate will my husband still be able to continue with his work permit or his also will get stuck with me as he initiated for 3 of us together
