In the US, at 65 years of age, you are eligible for Medicare. With this coverage and perhaps supplemental healthcare coverage, your medical expenses are paid for.

You can get international health insurance if you like. Medical care is much less expensive here than in the us, though, and many things are free at public clinics. Private care is available for those who prefer that. Pharmaceuticals are super cheap. Many also go to Mexico or Guatemala where it's even cheaper and they have more advanced equipment and facilities.
