Is there an English speaking Doctor that I can go to to get blood work done for routine check ups and to make sure I am healthy? And what is the price? Thanks.

Hi there 
luxmed - private medical care - is offering service called "profemed" which includes english-speaking doctors. You can phone them (+48 22 33 22 888), arrange a meeting, and pay with a credit card. After the visit you can have all your blood and other needed tests done. The visit cost ca.200 polish zloty.
