Hi guys am planning a short trip to Singapore for 5days. May I know the extra documents to add to my passport since its visa-free from Ghana(my country)

If Ghanaese are allowed to enter the country visa-free (or visa on arrival), you do not need to bring anything - but a return ticket (to show that and when you will leave again) would be helpful.
They will then give you a 30 or 15 days SVP (Social Visit Pass).
