Looking for a cousin moved to Germany

Hi all! About a year ago my cousin left Pakistan for Germany. My family hasn't heard anything about him for the latest four months. We suspect that his visa could be a forged one. According this document he might be at Ukrainian territory

Some of the officials of the Embassy made similar hints. Could it be so?

The link you gave does not work and your post is confusing about why you think he has a forged visa (for Germany, Ukraine, or ...?), why he should be in Ukraine and what the embassy (which embassy?) staff said.
What happens if you call his number in Germany or check at his address there? Could it be that he just doesn't want to contact you (which is perfectly legal)?
If he is missing, the police is the right authority to contact. Alternatively you can also check with the German registration office (Einwohnermeldeamt) at his residence whether he is still registered there.
