What are the formalities to obtain a birth certificate?

What is the procedure to follow if both parents are of the same nationality or for a mixed couple?

The hospital where the mother gave birth will forward the birth details to the nearest Civil Registry Office. Then within 14 or 30 days you need to claim a copy of the birth certificate in that office. If the parents are married, they need to bring a copy of marriage certificate (better if you have the polish version if you are married outside poland). They also need to bring their residence card and passport. Usually they will give you 3 copies of birth certificates in polish. I think an international version can be requested. That's all I know for now.
