there have been others asking about this form and plenty of responses pointing out that for an American resident to sign this form for someone they don't know from the internet is quite a big risk. I think you will have a hard time finding someone who will agree to give this to you on an internet forum.

There is a reason this paper is part of the application, and it's not considered "just paper" by the American goverment. It's legally binding after all.
